Airway orthodontics, maxillary expansion and nasal breathing

What is airway orthodontics, and how is it different from regular braces?

Airway orthodontics treats the jaw and dental arches as part of the breathing system, not just as a place to straighten teeth. Alongside standard alignment, it considers palate width, tongue posture and arch form because these directly affect the space available for nasal airflow. Regular orthodontics focuses mainly on tooth position and bite; airway orthodontics adds breathing and airway space as a treatment goal from the start.

Can orthodontic treatment help with nasal obstruction or mouth breathing?

It can help with the skeletal component. A narrow maxilla (upper jaw) forms a narrow nasal floor, which increases resistance to nasal airflow β€” widening it with maxillary expansion can improve nasal breathing in appropriately selected cases, particularly in growing children. It does not treat structural causes inside the nose itself, such as a deviated septum or enlarged turbinates β€” those need ENT assessment, ideally alongside the orthodontic evaluation.

What is maxillary expansion and who is it for?

Maxillary expansion is an appliance-based treatment that widens the upper jaw by gently opening the midpalatal suture, most effectively in children aged roughly 7–11 before that suture fuses. It is used for children with a narrow, high-arched palate, crowded teeth, or nasal breathing difficulty. Adults with a narrow maxilla can also be treated using MARPE or SARPE, though the process is more involved and requires careful imaging and planning.

My child has crowded teeth and also snores or breathes through their mouth β€” are these connected?

Often, yes. A narrow maxilla is frequently the shared cause: it reduces both dental arch width (leading to crowding) and nasal floor width (contributing to mouth breathing and snoring). When both are present together, it’s worth an assessment that looks at the crowding and the breathing as one picture rather than two unrelated complaints, ideally with an orthodontist and an ENT surgeon evaluating jointly.

Why does Rog Nidan involve both an orthodontist and an ENT surgeon for airway cases?

Airway problems usually have both a skeletal component (jaw and palate width, which orthodontics addresses) and a structural or soft-tissue component inside the nose itself (deviated septum, enlarged turbinates, adenoids, which need ENT evaluation). Treating only one half often leaves the other half of the problem unresolved. At what age should a child first be assessed for airway-related orthodontic concerns?

Around age 7, per the Indian Orthodontic Society’s recommendation for a first orthodontic check-up. This is well before all permanent teeth erupt, but it is the window in which a narrow palate, crossbites and early airway-related skeletal patterns can still be intercepted with simpler treatment, including rapid maxillary expansion, rather than waiting until the problem is fully established.
